Roles & Responsibilities

Provide top-notch home health physical therapy services to patients in Tehama County
Conduct start-of-care visits for orthopedic patients when nursing is not ordered
Manage an average daily census of 78 patients, with 5 visits per day
Collaborate with intake RNs and office managers for seamless care coordination
Use HCHB (HomeCare HomeBase) EMR for documentation
Travel within the service area (approximately 40 miles per day) to deliver care

Qualifications

Active Physical Therapist license in California (or compact eligible)
BLS certification required
At least 1 year of home health experience (willing to consider less with strong HH background)
Ability to start by June 16, 2026

Knowledge & Skills

Expertise in orthopedic and geriatric patient care
Proficiency with HCHB EMR system
Strong time management and independent work skills
Comfortable driving 40 miles per day within Tehama County
