Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM, rotating weekends and holidays

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Perform start of care evaluations, recertifications, and ongoing assessments for home health patients
Develop and implement individualized treatment plans for patients with CHF, orthopedic conditions, and pneumonia
Collaborate with clinical managers and educators for patient care guidance
Document patient progress and coordinate care with interdisciplinary team
Educate patients and families on therapeutic exercises and home safety

Qualifications

Active Physical Therapist license in Kentucky (compact eligible)
Current BLS certification (American Heart Association)
Minimum of 1 year of home health experience preferred; new grads welcome to apply
Valid driver's license and reliable transportation

Knowledge & Skills

Strong clinical skills in home health setting
Experience with OASIS documentation and home health regulations
Excellent communication and patient education abilities
Ability to work independently and manage a caseload efficiently
Familiarity with CHF, orthopedic rehab, and pneumonia management
