Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:30 AM - 5:00 PM, with rotating Saturdays once a month

Hours per Week: 40

Candidate Pay Rate: $58 per hour

Roles & Responsibilities

Assess and evaluate patients' physical therapy needs in a home health setting
Develop and update individualized treatment plans based on physician orders
Instruct patients and families on therapeutic exercises and equipment use
Monitor patient progress and adjust treatments as needed
Collaborate with physicians and other healthcare professionals to ensure comprehensive care
Document clinical summaries and progress notes accurately and timely

Qualifications

Minimum 2 years of experience as a Physical Therapist in acute, home care, or rehab setting
Current BLS certification required
Valid California PT license or willingness to obtain
Home health experience strongly preferred
Excellent communication and organizational skills

Knowledge & Skills

Strong assessment and treatment planning abilities
Knowledge of Medicare Conditions of Participation and Title 22 regulations
Proficiency in home health documentation and EHR systems
Ability to work independently and manage a caseload across Yolo County
