Location: Barre, Vermont

Contract Type: Travel Contract
Shift Type: Monday-Friday, Days (with weekend rotation approximately every 4th week)
Hours per Week: 36
Candidate Pay Rate: $67 per hour

Roles & Responsibilities
- Provide individualized, goal-oriented nursing care in patients' homes using the nursing process
- Serve as Case Manager for patients, coordinating care with physicians and multidisciplinary teams
- Develop and update care plans, monitor patient progress, and educate patients and families
- Administer medications, wound care, and other skilled nursing interventions
- Document all care accurately and timely in electronic health records
Qualifications
- Current and active Registered Nurse (RN) license in Vermont
- Minimum of 1 year of home health or related nursing experience
- Current BLS/BCLS certification (American Heart Association)
- Strong clinical assessment, critical thinking, and communication skills
Knowledge & Skills
- Expertise in home health nursing, case management, and patient education
- Ability to work independently and prioritize care in a home setting
- Familiarity with EHR systems and documentation standards
