Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide skilled physical therapy services to home health patients in a 60-mile radius around Valley City
Develop and implement individualized treatment plans for patients recovering from surgeries, falls, and other conditions
Collaborate with interdisciplinary teams to ensure comprehensive patient care
Document patient progress accurately using HomeCare/HomeBase EMR system
Educate patients and families on therapeutic exercises and home safety

Qualifications

Current North Dakota Physical Therapy License
Basic Life Support (BLS) certification from AHA
Minimum 1 year of physical therapy experience, preferably in home health
Valid driver's license and auto insurance required
Comfortable traveling within a 60-mile radius for patient visits

Knowledge & Skills

Strong clinical skills in physical therapy assessment and treatment
Experience with home health and/or hospice patient populations
Proficiency in HomeCare/HomeBase or similar EMR systems
Excellent communication and patient education skills
Ability to work independently without onsite supervision
