Shift Type: Monday-Friday, Days (flexible start times)

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Conduct comprehensive assessments and develop individualized treatment plans for home health patients
Provide skilled physical therapy interventions focusing on orthopedics and geriatric conditions
Collaborate with nurses, therapists, and physicians to ensure coordinated care
Educate patients and families on exercises and safety strategies for independence
Maintain accurate and timely documentation in HCHB (HomeCare HomeBase)
Perform start-of-care visits for orthopedic patients when nursing is not ordered

Qualifications

Active California Physical Therapy license required
BLS certification from the American Heart Association required
Home Health experience is a must - even for travelers with less than 2 years experience
Experience with HCHB or similar home health EMR strongly preferred
Ability to travel within Tehama County service area (approximately 40 miles daily)

Knowledge & Skills

Strong clinical skills in orthopedics and management of chronic comorbidities (CHF, COPD)
Knowledge of OASIS documentation and home health regulations
Excellent communication and interpersonal skills for patient and family education
Ability to work independently in a home setting with minimal supervision
Proficiency in electronic medical records, especially HomeCare HomeBase
