Roles & Responsibilities
- Provide compassionate, skilled nursing care to patients in their homes, delivering clinical excellence every visit
- Perform thorough patient assessments and monitor vital signs to guide care decisions
- Administer medications and treatments as ordered, ensuring patient safety and adherence
- Assist with wound care and chronic disease management to support healing and quality of life
- Educate patients and families on care plans and medications so they feel confident at home
- Maintain accurate and timely documentation in Cerner to ensure continuity of care
- Collaborate with the interdisciplinary healthcare team to deliver coordinated, patient-centered care
Qualifications
- Active California LVN license (required for submission)
- Minimum 2 years of nursing experience
- BLS certification (required)
- Home Health experience preferred
- Strong documentation and communication skills, Cerner experience preferred
- Able to manage time effectively and work independently in patient homes
Education
- Completion of an accredited vocational nursing program or equivalent (LVN)
Knowledge & Skills
- Patient assessment and vital sign monitoring
- Medication administration and treatment implementation
- Wound care and chronic disease management
- Proficient documentation practices, familiarity with Cerner preferred
- Patient and family education skills
- Time management and independent clinical decision-making in community settings
- Collaborative approach with interdisciplinary teams
