Shift Type: Monday-Friday, 8:00 AM - 4:30 PM (Day); full-time 40 hours/week with rotating weekends
Hours per Week: 40
Candidate Pay Rate: $41 per hour

Roles & Responsibilities
- Evaluate and assess patients to create individualized physical therapy plans
- Implement evidence-based treatment interventions to restore mobility and function
- Document patient progress accurately and communicate updates with the care team
- Collaborate with multidisciplinary staff at Lafayette Center / Powerback to coordinate care
- Educate patients and families on home programs and strategies for long-term recovery
- Participate in discharge planning and contribute to meaningful outcomes for patients
Qualifications
- Demonstrated experience in Physical Therapy (PT) practice
- Comfortable providing care in an outpatient/rehab-focused setting at the Lafayette Center / Powerback facility
- Strong clinical documentation and communication skills
- Ability to work the posted schedule (Monday–Friday with rotating weekends) and collaborate with a team
Knowledge & Skills
- Skilled in patient assessment and development of individualized treatment plans
- Proficient in therapeutic exercise, gait training, balance and functional mobility interventions
- Knowledge of documentation best practices and outcome tracking
- Excellent interpersonal skills and patient-centered approach
