Roles & Responsibilities

• Conduct comprehensive biopsychosocial assessments to identify patient needs and strengths
• Develop person-centered plans of care and collaborative discharge plans
• Provide timely crisis intervention and safety planning, including suicide screening
• Coordinate care and resources across disciplines and with community services
• Document thoroughly and accurately in Epic to support continuity of care
• Deliver brief, targeted short-term therapy and supportive counseling
• Complete required reports of harm per state statutes when necessary
• Participate in care conferences and collaborate with RN case managers to ensure safe discharges

Qualifications

• LMSW or LCSW required (LMSW or LCSW accepted; pending AK license is acceptable)
• Master of Social Work (MSW) preferred; degree must be from a CSWE-accredited program
• Minimum 1 year of clinical experience required; 2 years of acute care experience preferred
• Acute hospital/inpatient case management experience is required (hard stop)
• Experience with Epic charting preferred
• Ability and willingness to float between Emergency, Pediatric/Children's, and Adult teams
• Locals are not accepted — candidate permanent address must be at least 50 miles from the facility

Education

• Master of Social Work (MSW) from a CSWE-accredited program
• LMSW or LCSW licensure (Alaska license required or pending acceptable)

Knowledge & Skills

• Biopsychosocial assessment and psychosocial evaluation
• Discharge planning and care coordination
• Crisis intervention, safety planning, and suicide screening
• Knowledge of community resources (Medicaid, Medicare, housing, transportation)
• Short-term therapy techniques and brief clinical interventions
• Familiarity with NASW Code of Ethics and protective services reporting
• Comfort working with pediatrics, NICU/PICU, maternity, adults, and geriatrics
• Strong documentation skills and clinical communication
