Roles & Responsibilities

• Deliver skilled physical therapy services to skilled nursing residents with compassion and clinical excellence
• Assess functional status, develop individualized treatment plans, and track patient progress
• Document thoroughly and accurately to support clinical care and regulatory requirements
• Collaborate with nursing, rehab, and interdisciplinary team members to optimize outcomes
• Meet facility productivity expectations and contribute to a high-performing therapy program
• Adhere to Continuum Rehabilitation Group policies and procedures and facility guidelines
• Represent the therapy team professionally on-site and maintain required presence for guaranteed hours

Qualifications

• Licensed Physical Therapist (PT) or Doctor of Physical Therapy (DPT) — DPT listed as a primary skill
• 1-3 years of clinical experience (job lists 1-3 years)
• Experience or comfort working in a skilled nursing facility (SNF) setting
• Strong documentation and communication skills; ability to meet productivity targets
• Willingness to follow facility dress code (black scrubs) and on-site presence requirements

Education

• Doctor of Physical Therapy (DPT) preferred; entry-level Physical Therapy degree or equivalent expected

Knowledge & Skills

• Skilled in evaluation, therapeutic exercise, mobility training, and ADL-focused interventions
• Excellent clinical reasoning and ability to create practical, resident-centered plans of care
• Familiarity with SNF regulations, documentation expectations, and productivity metrics
• Team-oriented approach with strong communication and collaboration skills
• Professionalism in appearance and interactions with residents, families, and staff
