Roles & Responsibilities

• Provide high-quality outpatient physical therapy care for adolescents, adults, and geriatric patients
• Perform thorough musculoskeletal evaluations and assessments, including fall risk assessments
• Develop and implement individualized treatment plans for orthopedic and post-operative conditions
• Manage and build your caseload through screening, scheduling, and strong patient communication
• Deliver evidence-based interventions for conditions such as rotator cuff issues, ACL/meniscus injuries, low back pain, fractures, and total joint replacement
• Document care accurately and efficiently in NextGen EHR to support continuity and billing requirements
• Collaborate with case management and interdisciplinary teams to ensure coordinated patient care
• Meet productivity expectations (visits per day) while maintaining clinical quality and patient education

Qualifications

• Licensed Physical Therapist in Illinois or eligible for licensure (state license required)
• Minimum 1 year of outpatient physical therapy experience preferred
• Experience with orthopedic conditions and post-operative rehabilitation
• Proficiency with NextGen EHR or willingness to learn quickly
• Current BLS/CPR certification (or willingness to obtain)
• Strong documentation, case management, and time-management skills
• Comfort working toward clinic productivity expectations while delivering patient-centered care

Knowledge & Skills

• Musculoskeletal evaluation and orthopedic assessment skills
• Fall risk assessment and geriatric mobility expertise
• Rotator cuff non-surgical and post-operative rehabilitation
• Management of common orthopedic diagnoses: ACL tears, meniscus tears, low back conditions, fractures, osteoarthritis, and total joint replacement
• Skilled in building caseloads, patient screening, and efficient scheduling
• Familiarity with NextGen EHR workflows and clinical documentation best practices
• Strong patient communication, education, and discharge planning abilities
• Ability to meet productivity targets while maintaining high clinical standards
