Roles & Responsibilities
- Assess speech, language, swallowing, and cognitive-communication needs of SNF residents
- Develop and implement individualized, evidence-based treatment plans
- Provide skilled SLP interventions and skilled documentation in accordance with facility policies
- Collaborate with nursing, therapy colleagues, and interdisciplinary teams to support resident goals
- Maintain productivity expectations (therapist productivity requirement 90%) while delivering high-quality care
- Communicate progress and discharge needs with families and the care team
- Follow Continuum Rehabilitation Group (CRG) policies and facility procedures to ensure consistency of care
Qualifications
- 1-3 years of SLP experience (per job posting; 1 year minimum)
- Active SLP credentialing as required by state facility (state license and/or ASHA CCC-SLP as applicable)
- Experience or comfort working in Skilled Nursing Facility (SNF) practice settings
- Ability to be present in the building to receive guaranteed hours and provide on-site care
- Willingness to follow CRG policies and the facility dress code (black scrubs)
- Strong documentation skills and ability to meet productivity expectations
Knowledge & Skills
- Skilled in SLP assessments and interventions for adult/geriatric populations
- Knowledge of dysphagia management and safe swallowing strategies
- Proficiency with clinical documentation standards used in SNFs
- Excellent interpersonal and interdisciplinary communication skills
- Time management and caseload productivity awareness
- Ability to adapt treatment plans to each resident's goals and medical needs
