Roles & Responsibilities

• Provide compassionate, high-quality patient care in the Transitional Care Unit
• Assess and monitor patients, recognizing changes in condition and escalating appropriately
• Manage IVs/PICC lines and administer enteral feedings (NG/G-tube) safely and competently
• Perform wound care and manage Wound VAC devices with attention to best practices
• Care for chest tubes and tracheostomies, ensuring patient safety and comfort
• Document accurately and promptly in Meditech EMR and use unit equipment such as Pyxis
• Collaborate with multidisciplinary team members and float to Med-Surg and Surgical Pediatrics as needed
• Follow facility policies including flu vaccination requirements or approved exception processes

Qualifications

• Active Hawaii RN license required by the start of the assignment
• BLS (required)
• ACLS is preferred but not required for this unit (may be agency-dependent)
• 2–10 years of nursing experience (Transitional Care Unit or related Med-Surg experience preferred)
• Experience with Meditech EMR, Pyxis, wound VAC, chest tube and tracheostomy care, tube feedings, and PICC/PORT care
• Ability to float between units and remain flexible with shift assignments
• Proof of flu vaccination or approved medical/religious exception prior to contract start

Knowledge & Skills

• Strong patient assessment and monitoring skills
• IV/PICC line management and safe medication administration
• Enteral feeding and G-tube/NG tube management
• Wound care, including Wound VAC operation and dressing changes
• Chest tube and tracheostomy care
• Accurate EMR documentation (Meditech) and familiarity with Pyxis
• Effective communication, teamwork, and adaptability in a fast-paced unit
