Roles & Responsibilities

• Assess patients’ physical abilities and limitations to create compassionate, individualized care plans
• Develop and implement rehabilitative treatment programs that promote mobility and independence
• Monitor patient progress, adjust interventions as needed, and document outcomes clearly in the medical record
• Collaborate with physicians, nurses, and care teams to coordinate excellent, patient-centered care
• Educate patients and families on home exercise programs and strategies to support recovery
• Contribute to a positive team culture by mentoring colleagues and participating in quality improvement initiatives

Qualifications

• Licensed Physical Therapist in Pennsylvania (required) or ability to obtain state licensure promptly
• Graduation from an accredited physical therapy program (DPT preferred)
• BLS/CPR certification or willingness to obtain upon hire
• Strong clinical judgement, excellent communication skills, and a patient-first mindset
• Previous experience in outpatient or inpatient rehabilitation preferred but enthusiastic new grads are welcome

Education

• Graduation from an accredited Physical Therapy program (DPT preferred)

Knowledge & Skills

• Skilled in patient assessment, therapeutic exercise, and manual therapy techniques
• Proficient with documentation and electronic health record workflows
• Knowledge of rehabilitation principles, functional mobility training, and discharge planning
• Ability to educate patients and families in a clear, empathetic manner
• Strong teamwork, time management, and problem-solving abilities
