Location: Louisville, Kentucky

Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with rotating weekends

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Provide comprehensive physical therapy services including start of care, recertifications, and evaluations
Deliver personalized treatment plans for patients with CHF, orthopedic conditions, and pneumonia
Conduct daily home health visits to support patient recovery and independence
Collaborate with clinical managers and educators to ensure quality care delivery
Document patient progress and adjust treatment approaches as needed

Qualifications

Current Physical Therapist license required
BLS certification required
Home health experience preferred but not required
No new grads - experienced clinicians preferred
Valid driver's license and reliable transportation for home visits

Knowledge & Skills

Home health physical therapy expertise
Strong assessment and evaluation skills
Excellent communication and patient education abilities
Comfortable working independently in patient homes
Proficiency with electronic documentation systems
