Roles & Responsibilities

• Provide skilled physical therapy services to home health patients within a 60-mile radius of Valley City
• Develop and implement individualized treatment plans for patients recovering from surgeries, falls, and other conditions
• Collaborate with a multidisciplinary team to ensure coordinated care and optimal patient outcomes
• Perform comprehensive assessments and document patient progress using HomeCare/HomeBase EMR
• Educate patients and families on therapeutic exercises and safety strategies for independent living
• Manage a caseload of 5-7 patients per day, delivering compassionate care in the comfort of their homes

Qualifications

• Active North Dakota Physical Therapy License required
• Current BLS certification from the American Heart Association
• Minimum 1 year of previous home health experience
• Valid driver's license and auto insurance
• Comfortable traveling up to 60 miles for patient visits

Knowledge & Skills

• Expertise in physical therapy evaluation and treatment techniques
• Proficiency with HomeCare/HomeBase or similar home health EMR systems
• Strong clinical reasoning and problem-solving abilities
• Excellent communication and patient education skills
• Ability to work independently without on-site clinical manager supervision
• Time management skills to handle 5-7 patient visits per day
