Shift Type: Monday-Friday, 8:30 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $53 per hour

Roles & Responsibilities

Coordinate comprehensive patient care across the continuum, ensuring smooth transitions
Develop and implement individualized discharge plans to promote optimal outcomes
Collaborate with interdisciplinary teams including physicians, nurses, and social workers
Utilize Meditech and Midas EMR systems to document and track patient progress
Advocate for patients and families, connecting them with community resources

Qualifications

Active RN license in Florida (or compact license)
Minimum 1 year of case management experience
Proficiency with Meditech and Midas EMR systems required
Experience with complex discharge planning preferred

Knowledge & Skills

Strong knowledge of case management principles and discharge planning
Excellent communication and collaboration skills
Ability to manage a patient caseload of 25 or more
Familiarity with utilization review and insurance processes
