Contract Type: Travel Contract

Shift Type: Rotating schedule including weekends, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Conduct start of care, resumption of care, recertifications, evaluations, and daily visits for patients in a home health setting
Manage patients with CHF, orthopedic conditions, and pneumonia, tailoring treatment plans to individual needs
Collaborate with clinical managers and educators who provide 24/7 support for field clinicians
Document patient progress and adjust treatment plans to achieve optimal outcomes
Participate in weekend and holiday rotations as scheduled

Qualifications

Active Physical Therapist license in Kentucky
BLS certification required
Home health experience preferred, but new grads are welcome to apply
Valid driver's license and reliable transportation for home visits

Knowledge & Skills

Knowledge of home health regulations and OASIS documentation
Strong clinical assessment and critical thinking skills
Excellent communication and interpersonal abilities
Ability to work independently and manage a caseload efficiently
