Candidate Pay Rate: $53 per hour

Roles & Responsibilities

Conduct Oasis Assessments and Start of Care cases for home health patients
Provide physical therapy services for elderly patients with falls and fractures
Develop and implement individualized treatment plans
Use a tablet to track time and patient visits efficiently
Collaborate with a multidisciplinary team across four counties

Qualifications

Active Physical Therapist (PT) license in Indiana
Experience with Oasis Assessments and Start of Care cases preferred
Comfortable working in rural settings with mid-sized towns
Valid driver's license and reliable transportation

Knowledge & Skills

Strong knowledge of home health therapy practices
Excellent time management and documentation skills
Ability to work independently and travel between patient homes
Compassionate communication with patients and families
