Roles & Responsibilities

• Assess and evaluate patients' therapeutic, rehabilitative, and functional status, and develop individualized treatment plans
• Evaluate home environments to ensure safety and accessibility for therapy
• Instruct patients and families on the use of therapeutic appliances and exercises
• Report patient progress and reactions to treatment to the physician and adjust care plans accordingly
• Initiate and supervise physical therapy programs, including training patients in prosthetic device use
• Supervise Home Health Aides and homemaker services as part of the patient's rehabilitation regimen
• Identify patient needs for additional home health services and make appropriate referrals
• Prepare and submit clinical summaries and progress notes to document goal attainment
• Participate in peer consultations and staff competency assessments
• Comply with all regulatory requirements including Title 22, Medicare Conditions of Participation, and OSHA

Qualifications

• Minimum of 2 years of experience as a Physical Therapist in an acute, home care, or rehab setting
• Current BLS certification required
• Excellent oral and written communication skills
• Strong organizational skills
• Valid California PT license (or ability to obtain)
• Home health experience preferred

Knowledge & Skills

• In-depth knowledge of physical therapy principles and evidence-based practices
• Proficiency in home health care protocols and documentation
• Ability to assess muscle strength, mobility, gait, and range of motion
• Strong interpersonal skills to build rapport with patients and families
• Time management and ability to coordinate care across multiple patient visits
• Compassionate, patient-centered approach to care
