Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Deliver personalized physical therapy care to patients in their homes across Tehama County
Perform Start of Care visits especially for orthopedic patients when nursing is not ordered
Help patients recover mobility and independence after joint replacements and manage chronic conditions like CHF and COPD
Complete approximately 5 meaningful patient visits each day using a tablet and HCHB EMR
Collaborate with an Intake RN and Manager while enjoying autonomy in the field

Qualifications

Current Physical Therapist license in California
BLS certification required
Home Health experience preferred but will consider candidates with less than 2 years if they have Home Health experience
Valid driver's license and reliable transportation for field visits

Knowledge & Skills

Comfortable working independently in home settings
Familiarity with HomeCare HomeBase EMR is a plus
Strong communication and time-management skills
Ability to cover roughly 40 miles of driving per day within the service area
