Roles & Responsibilities

• Coordinate and manage patient-focused case management activities across the inpatient stay
• Develop and implement discharge plans that ensure safe transitions to post-acute care
• Document care, progress and plans accurately in the EMR (Meditech / Midas)
• Communicate clearly and compassionately with patients, families and the care team, including physicians
• Facilitate placement and transition to post-acute services and resources
• Drive throughput and efficiency by identifying barriers and collaborating on solutions
• Lead difficult conversations with empathy and clarity, advocating for the best patient outcomes

Qualifications

• Minimum 1 year bedside case management experience (explicit requirement)
• Experience with Meditech or HCA (explicit requirement)
• Knowledge of post-acute care and discharge planning (explicit requirement)
• Comfortable having hard conversations with patients, families and providers; strong communication skills
• Adaptable, proactive mindset and ability to work in a busy hospital environment
• Available for a 13-week travel contract

Education

• Active RN license (required)
• Nursing degree (ADN/BSN) or equivalent preferred

Knowledge & Skills

• Case management and discharge planning expertise
• Proficient with Meditech and Midas EMR systems
• Bedside case management and patient communication
• Understanding of post-acute care pathways and placement processes
• Throughput improvement experience
• Ability to collaborate with interdisciplinary teams
