Roles & Responsibilities

• Conduct thorough speech-language and swallowing evaluations for SNF residents
• Provide consistent, patient-centered daily treatments tailored to individual goals
• Plan and manage safe, timely discharges and effective transition plans
• Collaborate closely with interdisciplinary team members to optimize patient outcomes
• Document treatment, progress, and discharge plans accurately and on time
• Adhere to CRG policies & procedures and meet the facility productivity expectations

Qualifications

• 1-3 years of SLP experience (as listed in the job posting)
• Prior Skilled Nursing Facility (SNF) experience strongly preferred
• Ability to meet productivity expectations (90%) and follow facility policies
• Willingness to be onsite and follow site dress code (black scrubs) as required
• Comfortable working within an interdisciplinary therapy team

Knowledge & Skills

• Skilled in speech-language and swallowing evaluations
• Experience delivering daily treatment plans and tracking patient progress
• Proficient in discharge planning and care coordination
• Strong documentation and time-management skills
• Team-oriented communication and collaboration within a therapy department
• Familiarity with SNF workflows and regulatory expectations
