Roles & Responsibilities

• Conduct comprehensive evaluations to assess resident speech, language, and swallowing needs
• Provide daily treatments and individualized therapy interventions
• Plan and manage safe and effective discharge planning for residents
• Collaborate with the therapy team and facility staff to ensure continuity of care
• Document clinical progress and ensure productivity expectations are met
• Follow Continuum Rehabilitation Group policies and facility procedures
• Maintain professional appearance and dress code (black scrubs) while on shift

Qualifications

• 1-3 years of clinical experience (as indicated in the posting)
• Prior SNF experience strongly preferred
• Ability to follow facility and company policies, including productivity requirements (90%)
• Willingness to be present in the building to receive guaranteed hours
• Comfort working in a skilled nursing facility environment and collaborating with a rehab team
• Able to comply with facility dress code of black scrubs

Education

• Degree in Speech-Language Pathology (typical expectation: Master's degree preferred)

Knowledge & Skills

• Speech-language pathology evaluations and treatment planning
• Daily therapy delivery for SNF residents
• Discharge planning and coordination
• Documentation and productivity awareness in a contract setting
• Team-based rehabilitation and interdisciplinary communication
