Roles & Responsibilities

• Evaluate and provide skilled speech-language therapy to residents in a skilled nursing facility
• Develop individualized treatment plans and document progress clearly and professionally
• Collaborate with interdisciplinary team members to optimize patient-centered care
• Meet facility and CRG productivity expectations (90% productivity requirement)
• Follow CRG policies and procedures and maintain professional standards (dress code: black scrubs)
• Communicate with families and facility staff to support continuity of care
• Be present on-site to receive guaranteed hours and contribute to the therapy team culture

Qualifications

• Current credentialing as a Speech-Language Pathologist (state licensure or credential required per facility standards)
• 1-3 years of experience working in skilled nursing facility (SNF) or similar settings preferred
• Strong competency with therapy documentation and meeting productivity expectations
• Comfort working on-site and following facility dress code and policies
• Excellent communication and teamwork skills to collaborate with nursing and rehab staff

Education

• Degree in Speech-Language Pathology (or related graduate credential) and appropriate state licensure or certification

Knowledge & Skills

• Skilled Nursing Facility (SNF) practice experience and familiarity with long-term care workflows
• Solid documentation skills and knowledge of regulatory expectations for therapy services
• Productivity management and time prioritization to meet a 90% productivity target
• Ability to assess and treat common speech, language, swallowing, and cognitive-communication disorders
• Strong interpersonal skills for working with residents, families, and multidisciplinary teams
