Roles & Responsibilities

• Provide skilled speech-language pathology services to residents in a skilled nursing facility
• Perform thorough assessments and create individualized, evidence-based treatment plans
• Deliver hands-on therapy and track resident progress with clear documentation
• Collaborate closely with nursing, rehab staff, and families to optimize resident outcomes
• Follow Continuum Rehabilitation Group (CRG) policies and facility procedures
• Meet productivity expectations and maintain professional appearance (black scrubs required)

Qualifications

• Licensed Speech-Language Pathologist (SLP) or eligible for New Mexico licensure
• 1-3 years of clinical experience (experience in SNF/long-term care is a strong plus)
• Strong skills in assessment, treatment planning, and documentation for typical SNF caseloads
• Ability to work collaboratively on an interdisciplinary team and communicate effectively
• Reliable, professional presence onsite to receive guaranteed hours and fulfill schedule commitments

Education

• Graduate degree in Speech-Language Pathology (Master's or clinical doctorate preferred)

Knowledge & Skills

• Knowledge of skilled nursing facility therapy practices and SNF documentation standards
• Familiarity with productivity expectations and CRG policies & procedures
• Skilled in clinical reasoning for dysphagia, cognitive-communication, language, and voice (common SNF needs)
• Comfortable with time management, caseload prioritization, and progress tracking
