Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in a long-term care setting
• Administer medications and treatments accurately and safely according to policy
• Perform thorough patient assessments and monitor changes in condition
• Document care and observations clearly in the medical record
• Collaborate with nurses, providers, and interdisciplinary team members to coordinate resident care
• Support residents and families with clear communication and respectful education
• Follow infection control practices and safety protocols to protect residents and staff
• Participate in quality improvement initiatives to enhance resident outcomes

Qualifications

• Licensed Practical Nurse (LPN) — State of Hawaii license required by start of assignment (or ability to obtain prior to start)
• Experience working in long-term care (LTC) settings is required
• Comfortable working 12-hour shift patterns
• Strong communication skills and a team-focused attitude
• Able to pass required background checks and facility onboarding

Education

• Graduate of an accredited Practical Nursing (LPN/LVN) program
• Active LPN licensure (State of Hawaii) required by assignment start

Knowledge & Skills

• Knowledge of long-term care nursing practices and geriatric care
• Medication administration and documentation accuracy
• Skilled in basic patient assessments and monitoring
• Familiarity with infection control and safety procedures
• Good time management, prioritization, and teamwork skills
• Comfortable using electronic medical records and completing clear charting
