Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in a skilled nursing/rehabilitation setting
• Perform thorough assessments and document resident status, changes, and care plans with accuracy
• Administer medications and treatments safely and according to facility protocols
• Collaborate with the interdisciplinary team — therapists, social services, and physicians — to deliver coordinated care
• Support residents with activities of daily living while promoting independence and dignity
• Respond promptly to resident needs and emergency situations with calm, competent action
• Communicate clearly with families and caregivers to keep them informed and involved in care

Qualifications

• Current and active LPN license in Massachusetts (required)
• Previous experience in long-term care or rehabilitation preferred but new grads with strong clinical skills are welcome
• BLS/CPR certification (American Heart Association or equivalent) — or willingness to obtain upon hire
• Ability to pass facility background checks and meet health screening requirements
• Strong communication skills and a team-oriented approach

Education

• Graduate of an accredited practical nursing program (LPN diploma or certificate)

Knowledge & Skills

• Proficient in medication administration and documentation practices
• Skilled in wound care basics, vital signs monitoring, and ADL assistance
• Knowledge of infection control standards and resident safety protocols
• Comfortable working independently on night shifts and escalating when necessary
• Good clinical judgment, time management, and clear charting skills
