Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents in a skilled nursing/rehabilitation setting
- Perform thorough assessments and document resident status, changes, and care plans with accuracy
- Administer medications and treatments safely and according to facility protocols
- Collaborate with the interdisciplinary team — therapists, social services, and physicians — to deliver coordinated care
- Support residents with activities of daily living while promoting independence and dignity
- Respond promptly to resident needs and emergency situations with calm, competent action
- Communicate clearly with families and caregivers to keep them informed and involved in care
Qualifications
- Current and active LPN license in Massachusetts (required)
- Previous experience in long-term care or rehabilitation preferred but new grads with strong clinical skills are welcome
- BLS/CPR certification (American Heart Association or equivalent) — or willingness to obtain upon hire
- Ability to pass facility background checks and meet health screening requirements
- Strong communication skills and a team-oriented approach
Education
- Graduate of an accredited practical nursing program (LPN diploma or certificate)
Knowledge & Skills
- Proficient in medication administration and documentation practices
- Skilled in wound care basics, vital signs monitoring, and ADL assistance
- Knowledge of infection control standards and resident safety protocols
- Comfortable working independently on night shifts and escalating when necessary
- Good clinical judgment, time management, and clear charting skills
