Roles & Responsibilities
- Provide compassionate, high-quality nursing care to residents as a licensed practical nurse
- Administer medications and treatments accurately and document them thoroughly
- Monitor resident condition and report changes promptly to the interdisciplinary team
- Collaborate with RNs, physicians, and therapy staff to implement individualized care plans
- Assist with admissions, discharges, and transfers to ensure smooth transitions
- Educate residents and families about care plans and daily routines to promote comfort and understanding
- Maintain accurate, timely charting and follow facility policies and regulatory standards
Qualifications
- Current Licensed Practical Nurse (LPN) license — active and in good standing
- T-LPN classification indicated (as required by the facility)
- Comfortable working overnight shifts and providing weekend coverage every other week
- Ability to pass required background check and drug screen per facility policy
- Strong communication and teamwork skills, with a patient-centered attitude
Education
- Graduation from an accredited LPN/LVN program and possession of current state LPN licensure
Knowledge & Skills
- Solid fundamentals in medication administration and resident assessment
- Proficiency in documentation and electronic charting systems (facility-specific orientation provided)
- Skills in wound care, ADL assistance, and basic clinical procedures
- Comfortable working in long overnight shifts and prioritizing patient safety
- Good clinical judgment and the ability to escalate concerns appropriately
