Roles & Responsibilities

• Provide compassionate direct patient care including medication administration and monitoring
• Monitor and record vital signs, patient status, and changes in condition
• Document care accurately and timely in the facility charting system
• Collaborate with RN and interdisciplinary team members to support care plans
• Assist patients with activities of daily living while promoting dignity and independence
• Contribute to a safe, clean environment by following infection control and safety protocols
• Communicate clearly and empathetically with patients and families to support their needs

Qualifications

• Current Licensed Practical Nurse (LPN) credential required
• Travel LPN or T-LPN experience is a plus and fits this assignment well
• Active license to practice in the state of New Mexico or eligibility to obtain one
• Ability to work scheduled 12+ hour day shifts including weekends
• Strong communication skills and a team-first attitude

Education

• Graduate of an accredited LPN or practical nursing program; high school diploma or GED required

Knowledge & Skills

• Clinical skills in medication administration, wound care basics, and patient assessment
• Comfortable taking and interpreting vital signs and reporting changes
• Accurate clinical documentation and familiarity with electronic health records
• Basic knowledge of infection control practices and patient safety principles
• Good time management, prioritization, and teamwork in a fast-paced setting
