Roles & Responsibilities

• Provide compassionate, resident-centered nursing care in a skilled nursing setting
• Assess residents, monitor condition changes, and promptly communicate concerns to the care team
• Administer medications and treatments safely and document accurately
• Assist residents with activities of daily living while promoting independence and dignity
• Collaborate with interdisciplinary team members to create and follow individualized care plans
• Maintain accurate clinical records in the facility charting/EMR system
• Adhere to infection control, safety, and facility policies to protect residents and staff

Qualifications

• Current, active LPN or RN license in Massachusetts (required)
• BLS/CPR certification (required or obtain upon hire)
• Previous experience in long-term care or skilled nursing preferred
• Up-to-date immunizations and health screenings per facility policy
• Strong communication, time management, and teamwork skills
• Comfortable working per diem shifts and adapting to varied assignments

Education

• LPN: Diploma or certificate in Practical Nursing
• RN: Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN)

Knowledge & Skills

• Patient assessment and documentation skills
• Medication administration and safe practices
• Familiarity with infection control principles and wound care basics
• Comfortable using electronic charting / EMR systems
• Ability to prioritize tasks in a fast-paced skilled nursing environment
• Compassionate, patient-focused approach to care
