Roles & Responsibilities

• Assess and evaluate patients in their homes to develop personalized treatment plans
• Deliver skilled therapeutic interventions to improve mobility, function, and safety
• Educate patients and caregivers on home exercise programs and fall prevention
• Coordinate care with the interdisciplinary team and community resources
• Document accurate, timely clinical notes (Epic preferred) and track outcomes
• Maintain compliance with agency policies, licensure requirements, and required competencies

Qualifications

• Active Physical Therapist license (able to practice in Iowa) or eligibility for licensure
• BLS (Basic Life Support) required
• Home health experience or strong interest in home-based care
• Comfortable with in-home assessments and independent clinical decision-making
• Completion of Trinity specialty competency testing and Trinity skills checklist (per checklist)
• Epic experience preferred but not required

Education

• Graduation from an accredited Physical Therapy program (DPT preferred)

Knowledge & Skills

• Strong knowledge of musculoskeletal and neurological rehabilitation principles
• Experience with home safety assessments and adaptive equipment recommendations
• Skilled in therapeutic exercise prescription, gait training, and functional mobility
• Clear patient and caregiver education skills with a focus on outcomes
• Proficient documentation practices and basic familiarity with Epic (preferred)
• Good infection control practices and adherence to vaccination/health requirements
