Roles & Responsibilities

• Provide direct patient care and deliver rehabilitation treatments as directed by the Physical Therapist
• Record daily treatment notes and weekly progress notes in compliance with PT board and facility policies
• Implement physician and therapist treatment plans to support patient goals and safe discharge
• Assist nursing staff by training Restorative Aides and educating caregivers for continuity of care
• Participate in patient care and rehabilitation conferences to coordinate interdisciplinary plans
• Maintain a safe, clean, and organized treatment area and report any equipment issues promptly
• Lead or participate in in-service training sessions to raise team clinical skills and standards
• Communicate clearly with supervisors, families, and team members about patient progress and concerns

Qualifications

• Licensed Physical Therapist Assistant in the state (must maintain license in good standing)
• Proficient computer skills and experience inputting clinical data into Casamba and PointClickCare
• Strong documentation skills with attention to regulatory and PT board requirements
• Experience with caregiver training, restorative aide instruction, and rehabilitation interventions
• Excellent communication skills and the ability to relate positively with patients, families, and staff
• Willingness to participate in facility in-service programs and continuing education

Education

• Licensed as a Physical Therapist Assistant by the state board (state licensure required and must be maintained)

Knowledge & Skills

• Direct patient care and hands-on rehabilitation techniques
• Clinical documentation and progress note preparation
• Patient and caregiver education for discharge readiness
• Implementation of PT treatment plans and therapeutic modalities
• Familiarity with Casamba and PointClickCare workflows
• Ability to train restorative aides and provide in-service education

• Strong interpersonal and team communication skills
