Roles & Responsibilities

• Assess and evaluate patients across settings including home health and outpatient to create individualized treatment plans
• Deliver evidence-based therapeutic interventions to improve mobility, function, and quality of life
• Coordinate care with interdisciplinary teams, family members, and caregivers to support patient goals
• Document patient progress accurately and timely in the electronic medical record
• Educate patients and families on home exercise programs, fall prevention, and safe mobility techniques
• Adapt treatment plans based on patient response and communicate changes to the care team

Qualifications

• Licensed Physical Therapist in Nevada or eligible for Nevada licensure
• Current BLS/CPR certification
• Experience caring for patients in Medical/Surgical, Telemetry, Long Term Care, Outpatient, or Home Health settings preferred
• Strong clinical reasoning, assessment, and documentation skills
• Excellent communication and teamwork skills to collaborate with multidisciplinary teams
• Willingness to travel and adapt to varied patient care locations

Education

• Doctor of Physical Therapy (DPT) or Master of Physical Therapy (MPT) required or equivalent

Knowledge & Skills

• Skilled in patient assessment, therapeutic exercise prescription, and functional mobility training
• Knowledgeable about home safety evaluations and adaptive equipment recommendations
• Proficient with documentation and electronic medical records
• Ability to prioritize caseloads and manage time across home and outpatient visits
• Strong patient education and care coordination abilities
