Roles & Responsibilities
- Assess and evaluate patients across settings including home health and outpatient to create individualized treatment plans
- Deliver evidence-based therapeutic interventions to improve mobility, function, and quality of life
- Coordinate care with interdisciplinary teams, family members, and caregivers to support patient goals
- Document patient progress accurately and timely in the electronic medical record
- Educate patients and families on home exercise programs, fall prevention, and safe mobility techniques
- Adapt treatment plans based on patient response and communicate changes to the care team
Qualifications
- Licensed Physical Therapist in Nevada or eligible for Nevada licensure
- Current BLS/CPR certification
- Experience caring for patients in Medical/Surgical, Telemetry, Long Term Care, Outpatient, or Home Health settings preferred
- Strong clinical reasoning, assessment, and documentation skills
- Excellent communication and teamwork skills to collaborate with multidisciplinary teams
- Willingness to travel and adapt to varied patient care locations
Education
- Doctor of Physical Therapy (DPT) or Master of Physical Therapy (MPT) required or equivalent
Knowledge & Skills
- Skilled in patient assessment, therapeutic exercise prescription, and functional mobility training
- Knowledgeable about home safety evaluations and adaptive equipment recommendations
- Proficient with documentation and electronic medical records
- Ability to prioritize caseloads and manage time across home and outpatient visits
- Strong patient education and care coordination abilities
