Roles & Responsibilities

• Provide skilled physical therapy interventions in patients' homes to improve function and independence
• Perform thorough evaluations and develop individualized care plans tailored to home health needs
• Manage a caseload of approximately 5-6 patient visits per day while maintaining quality and safety
• Document visits, progress notes and plans accurately and promptly using HCHB
• Coordinate care and communicate effectively with nurses, caregivers and the interdisciplinary team
• Educate patients and families on home exercise programs, fall prevention and safe mobility
• Prioritize timely travel between visits and maintain professional standards and appearance

Qualifications

• Licensed Physical Therapist (PT) in North Carolina or eligible for state licensure
• Minimum 2 years of clinical experience, with home health experience required for therapy positions
• Experience using HCHB (HomeCare HomeBase) documentation system
• Reliable vehicle and valid driver’s license for patient visits across Franklin/Macon County
• Strong communication skills and the ability to work independently in community settings

Education

• Doctor of Physical Therapy (DPT) or Master's in Physical Therapy or equivalent

Knowledge & Skills

• Home health clinical assessment and skilled PT interventions
• Care planning and functional outcome measurement
• Proficient documentation in HCHB
• Time management, route planning and efficient visit scheduling
• Patient and caregiver education, safety evaluation and fall prevention strategies
• Ability to collaborate with interdisciplinary teams and adjust care based on patient needs
