Roles & Responsibilities

• Assess patients' mobility, function and pain levels to develop individualized care plans
• Design and implement evidence-based therapeutic interventions to restore movement and independence
• Document patient progress clearly and timely in the facility's charting system
• Collaborate with nursing, physicians and therapy team members to coordinate care
• Educate patients and caregivers on home programs, fall prevention and safe mobility strategies
• Participate in interdisciplinary meetings and contribute to quality improvement efforts
• Ensure compliance with regulatory, safety and documentation standards at all times

Qualifications

• Licensed Physical Therapist (PT) — ability to practice in Maine or eligibility to obtain licensure
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Comfortable working in a rehabilitation/contract facility setting
• Strong clinical reasoning and documentation skills
• Professional, team-oriented attitude and reliable attendance for scheduled shifts
• Credentials and immunizations as required by the facility (completed prior to start)

Education

• Doctor of Physical Therapy (DPT) or Bachelor's in Physical Therapy (or equivalent clinical degree)

Knowledge & Skills

• Knowledge of therapeutic modalities, mobility training and functional rehabilitation
• Skilled in developing individualized treatment plans and measurable goals
• Proficient with clinical documentation and basic electronic medical records
• Effective communication with patients, families and interdisciplinary teams
• Time management and ability to prioritize caseload for efficient, high-quality care
