Roles & Responsibilities

• Provide skilled physical therapy services in patients' homes following a physician's plan of care
• Assess, evaluate, and update therapeutic/rehabilitative/functional status (mobility, gait, ROM) and contribute to the total plan of care
• Evaluate the home environment and identify safety or equipment needs to maximize patient independence
• Develop and implement individualized treatment plans and direct PT treatments
• Prepare and submit clinical progress summaries and timely documentation (OASIS experience required)
• Educate patients and caregivers on therapeutic exercises, appliance use, and safety strategies
• Supervise Home Health Aides and homemakers when they participate in the patient’s rehabilitation program
• Participate in case conferences, peer consultation, and ongoing staff competency assessments
• Comply with Medicare/Medi-Cal, Title 22, OSHA and other regulatory/licensure requirements

Qualifications

• Current California Physical Therapy license required
• BLS certification required
• NPI number required
• Minimum 2 years of experience in a healthcare setting (home health/community experience preferred)
• Home Health experience including OASIS required; HomeCare Homebase (HCHB) EMR preferred
• Current driver’s license and proof of automobile insurance required
• Ability to provide two supervisor references at submission (one within 12 months and one within 3 years)
• Candidates must not have been employed by any CommonSpirit/CHI/Dignity facility within the past year (PRN through outside firms may be considered)

Knowledge & Skills

• Strong patient assessment and clinical documentation skills (OASIS familiar)
• Expertise in mobility, gait, ROM assessment and therapeutic treatment planning
• Home environment evaluation and safety planning
• Clear oral and written communication and strong organizational skills
• Experience supervising and mentoring Home Health Aides
• Familiarity with HomeCare Homebase (HCHB) EMR is a plus
• Understanding of Medicare/Medi-Cal, Title 22, and OSHA regulatory requirements
