Roles & Responsibilities

• Evaluate patients comprehensively to identify impairments and functional limitations
• Develop and implement individualized treatment plans to improve strength, range of motion, balance, safety and function
• Deliver therapeutic exercise and manual interventions appropriate to outpatient diagnoses
• Teach patients and caregivers effective home exercise programs and safety strategies
• Document treatments, progress and discharge plans clearly and promptly in the medical record
• Collaborate with the interdisciplinary rehab team to shorten patient wait times and optimize throughput
• Contribute to a patient-centered environment that prioritizes measurable outcomes and patient education

Qualifications

• Bachelor's degree in Physical Therapy (as specified by the client)
• Current Colorado PT licensure (DORA) or eligibility via the PT compact for Colorado
• Minimum 2 years of PT experience required; 3–5 years preferred
• Current BLS certification through the American Heart Association
• Proof of current season flu vaccination (no declinations accepted during required period)
• Ability to work 4x10-day schedule with a Saturday rotation and handle typical outpatient caseload and documentation
• Submit resume, skills sheet, BLS (AHA), Colorado PT license or compact evidence, and proof of current flu vaccine with application

Education

• Bachelor's degree in Physical Therapy

Knowledge & Skills

• Strong patient evaluation and differential diagnosis skills
• Skilled in developing and progressing treatment plans for outpatient diagnoses
• Proficiency with therapeutic exercise, ROM restoration and balance training
• Experience prescribing and teaching effective home exercise programs
• Excellent patient education and caregiver coaching abilities
• Solid documentation and time management skills in an outpatient EMR environment
• Team-oriented communication and the ability to adapt to variable weekday schedules including Saturdays
