Roles & Responsibilities
- Provide skilled physical therapy treatments under the supervision of a licensed Physical Therapist
- Implement individualized plans of care using therapeutic exercise and appropriate modalities in patients' homes
- Assess and document patient progress clearly and timely, ensuring care plans are followed
- Educate patients and caregivers to promote independence and safe home function
- Coordinate with nurses, social workers and other interdisciplinary team members to support holistic care
- Maintain professional standards, keep licenses and BLS current, and submit required documentation for verification
- Travel safely to patient homes throughout a northern territory and adapt care to each home environment
Qualifications
- Active New Hampshire Physical Therapist Assistant practice license (Compact allowed)
- Current American Heart Association BLS (required)
- 1-2 years recent PTA experience in Home Health (required)
- Home Health and/or Hospice experience (required)
- Ability to provide supervisory signed reference from past year (required)
- Own reliable vehicle and comfortable driving in winter conditions
- License verification and current documentation at time of submission
Knowledge & Skills
- Therapeutic exercise and use of modalities in home settings
- Plan of care implementation and adherence to PT supervision
- Experience treating patients across the lifespan: pediatric to geriatric
- Strong documentation, communication and patient education skills
- Time management and ability to work independently in the field
- Comfortable navigating rural routes and adapting care to the home environment
