Roles & Responsibilities

• Plan and execute individualized rehabilitation programs to help patients regain function and independence
• Perform thorough patient evaluations and develop appropriate treatment plans per physician prescription
• Deliver therapeutic exercise, neuromuscular re-education, gait training, manual therapy, and physical modalities
• Motivate and educate patients using effective communication and compassionate care to maximize outcomes
• Document assessments, progress, and treatment plans accurately and timely
• Assist in clinic management and help supervise personnel and students to maintain high standards
• Collaborate with referring physicians and interdisciplinary teams to coordinate patient care
• Contribute to a patient-centered culture and continuous improvement initiatives

Qualifications

• 1+ years of Physical Therapy experience (required)
• Outpatient orthopedics experience (required)
• Active Illinois Physical Therapist license (must be active)
• Active American Heart Association BLS certification (required)
• Current skills checklist relevant to Physical Therapist specialty (within 1 year)
• Two supervisory reference forms with dates employed (required)
• Candidates must provide permanent address; applicants within a 50 mile radius of the facility are not eligible
• Applicants who were direct employees within the last 2 years are not eligible; facility will run name clear checks as needed
