Roles & Responsibilities

• Provide thorough PT evaluations and create personalized treatment plans that help patients return to the activities they love
• Perform fall risk assessments and implement prevention strategies to keep patients safe and independent
• Deliver orthopedic rehabilitation and post-operative care, including total joint replacement recovery
• Manage a focused caseload and productivity expectations (target visits per day and 25 active patients)
• Screen and build your caseload by triaging referrals and coordinating care with the team
• Document accurately and efficiently in the NextGen EHR so patient care and billing stay on track
• Collaborate with case management and interdisciplinary partners to optimize patient outcomes

Qualifications

• Licensed Physical Therapist in Michigan or eligible for Michigan licensure
• Basic Life Support (BLS/CPR) certification (or willing to obtain)
• Comfortable with outpatient orthopedic populations and post-operative care
• Experience with PT evaluation, fall risk assessment, and case management preferred
• Ability to manage visits-per-day expectations (typical target 8-10) and maintain a 25 active patient caseload
• Open to new graduates; 1+ year of outpatient experience preferred
• Proficiency or willingness to learn NextGen EHR
