Roles & Responsibilities
- Assess patients' mobility, strength, balance, and functional needs to build personalized care plans
- Develop and implement therapeutic interventions that promote independence and safe function
- Collaborate with nurses, aides, physicians, and family members to coordinate comprehensive care
- Document progress clearly and timely, ensuring continuity of care and regulatory compliance
- Educate patients and caregivers on home exercise programs and safety strategies
- Monitor outcomes and adjust treatment plans to maximize patient progress
- Serve as a patient advocate, promoting dignity and quality of life in the SNF setting
Qualifications
- DPT or degree in Physical Therapy (PT) — new grads are welcome and encouraged to apply
- Active or eligible for Pennsylvania state PT license
- Current Basic Life Support (BLS) certification preferred or willing to obtain
- Comfort working in a skilled nursing facility (SNF) environment with interdisciplinary teams
- Strong communication skills and a patient-centered approach to care
Education
- Doctor of Physical Therapy (DPT) or equivalent PT degree
- New graduates accepted
Knowledge & Skills
- Knowledge of geriatric rehabilitation, transfers, gait training, and fall prevention
- Skill in evaluating functional mobility and creating practical, goal-oriented plans of care
- Ability to document per facility and regulatory standards
- Comfortable with time management and treating multiple patients in a busy SNF caseload
- Teamwork mindset and ability to educate staff and family members
