Roles & Responsibilities

• Assess residents' functional needs and design individualized occupational therapy plans
• Implement therapeutic interventions to improve daily living skills and independence
• Collaborate with nurses, PTs, SLPs and the interdisciplinary team to coordinate care
• Document progress and goals clearly and timely in the facility’s clinical record
• Educate residents and caregivers on safe strategies, adaptive equipment and home programs
• Participate in team meetings and care planning to advocate for patient-centered outcomes
• Monitor outcomes and modify treatment plans to maximize resident progress and safety

Qualifications

• Any experience level welcome—including new grads (job open to clinicians at all stages)
• Licensed Occupational Therapist in Oregon or eligible for Oregon licensure (facility will verify credentialing)
• Current basic life support (BLS) or CPR certification as required by employer/policy
• Comfort working in a Skilled Nursing Facility (SNF) setting and with complex geriatric cases
• Strong communication, documentation and teamwork skills

Education

• Open to all education levels; new graduates welcome (facility did not specify a minimum degree)

Knowledge & Skills

• Knowledge of occupational therapy evaluation and intervention across ADLs/IADLs
• Skilled in care planning and progress documentation for SNF residents
• Familiarity with mobility training, adaptive equipment, seating and positioning
• Ability to work with interdisciplinary teams and contribute to discharge planning
• Competence with electronic health records and timely charting
