Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:30 AM - 5:00 PM, occasional Saturdays

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Provide life-changing physical therapy services to patients in the comfort of their own homes.
Assess and evaluate functional status, develop personalized treatment plans, and educate families on therapeutic techniques.
Collaborate with physicians and healthcare teams to ensure optimal patient outcomes.
Supervise home health aides and document progress to track recovery milestones.
Make a difference in the Yolo County community by helping patients regain independence.

Qualifications

Active Physical Therapist license in California
BLS certification required
Minimum 2 years of experience in acute care, home health, or rehabilitation settings
Valid driver's license and reliable transportation
Excellent communication and organizational skills

Knowledge & Skills

Strong clinical assessment skills
Knowledge of home health regulations (Title 22, Medicare, Medi-Cal)
Ability to work independently and manage a caseload
Proficiency in electronic health records
