Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Conduct start of care assessments, recertifications, and resumption of care for home health patients
Develop and implement individualized physical therapy plans to improve mobility and quality of life
Collaborate with a supportive team of clinical coordinators and fellow therapists
Document patient progress accurately and efficiently using OASIS
Provide patient and family education on therapeutic exercises and safety

Qualifications

Current ND Physical Therapy License (required)
BLS certification from the American Heart Association
Valid Driver's License and proof of Auto Insurance
Home Health experience strongly preferred
First-time travelers welcome to apply!
Ability to meet productivity requirements of 6.5 points per day

Knowledge & Skills

Proficiency in OASIS documentation and home health regulations
Excellent communication and interpersonal skills
Comfortable using electronic medical records and scheduling systems
Ability to work independently and manage a daily caseload effectively
