End Date: 09-28-2026

Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM; rotating weekends and holidays

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Perform start of care, resumption of care, recertifications, and evaluations for home health patients
Provide ongoing daily visits and adjust care plans as needed
Collaborate with clinical managers and educators for support and guidance
Document patient progress and communicate with interdisciplinary team
Cover assigned geographic area in Louisville and surrounding communities

Qualifications

Active Physical Therapy license in Kentucky (or compact eligible)
BLS certification required
Home health experience preferred but not required; new grads welcome to apply
Ability to work independently and manage a caseload

Knowledge & Skills

Strong clinical assessment and critical thinking skills
Knowledge of home health regulations and OASIS documentation
Experience with common diagnoses: CHF, ortho, pneumonia
Excellent communication and time management abilities
