Candidate Pay Rate: $53 per hour

Roles & Responsibilities

Conduct comprehensive Oasis Assessments and Start of Care visits for home health patients
Manage a caseload of 4-6 patient visits daily using a point system for efficient scheduling
Provide skilled physical therapy interventions for common cases such as elderly falls and fractures
Serve patients across four counties including Noble, Lagrange, Steuben, and DeKalb
Utilize a tablet to accurately track time and visits for documentation

Qualifications

Active Physical Therapist license in the state of Indiana
Experience with Oasis Assessments and Start of Care is preferred
Comfortable working in rural and mid-sized town settings
Reliable transportation to travel between patient homes across multiple counties

Knowledge & Skills

Proficient in conducting Oasis Assessments and developing care plans
Strong clinical skills in managing orthopedic and geriatric conditions
Excellent time management and ability to handle a daily point-based workload
Familiarity with electronic documentation using tablets
