Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide skilled physical therapy services to home health patients in the Valley City area
Develop individualized treatment plans to help patients regain mobility and independence
Educate patients and families on exercises, safety, and fall prevention
Coordinate care with interdisciplinary team including nurses and therapists
Document patient progress accurately in HomeCare/HomeBase EMR
Travel within a 60-mile radius to deliver care in patients' homes

Qualifications

Current North Dakota Physical Therapy License
BLS certification (American Heart Association)
Valid driver's license and auto insurance
Minimum 1 year of home health experience
Previous travel experience not required
Comfortable using HomeCare/HomeBase EMR

Knowledge & Skills

Strong clinical assessment and treatment planning skills
Knowledge of home health regulations and Medicare guidelines
Excellent communication and patient education abilities
Ability to work independently and manage a caseload of 5-7 patients per day
Proficient in electronic medical records
