Roles & Responsibilities

• Provide skilled physical therapy assessments and individualized treatment plans in patients’ homes
• Deliver hands-on mobility, balance, and functional training to help patients regain independence
• Collaborate with interdisciplinary care teams and communicate progress clearly
• Document clinical findings and treatment in the EMR (Epic preferred) with timely, accurate notes
• Perform Trinity Specialty Specific Competency Testing and follow Trinity Skills Checklist
• Ensure patient safety by completing required screenings, fit tests, and infection-prevention protocols

Qualifications

• Valid Physical Therapy license (license verification required prior to start)
• BLS certification (required)
• Trinity Specialty Specific Competency Testing and completion of Trinity Skills Checklist
• Current immunizations and screenings per employer requirements (MMR, Hep B, TB test or IGRA, Varicella, Tdap, COVID-19, Influenza, drug screen, fit test)
• Comfortable and experienced delivering care in home health settings
• Epic experience preferred but not required

Knowledge & Skills

• Strong patient assessment and treatment planning skills in a home environment
• Skilled in therapeutic exercise, gait training, transfers and mobility aids
• Excellent clinical documentation and electronic charting (Epic preferred)
• Ability to prioritize safety, infection control, and patient-centered care
• Good communication and collaboration with nursing, MDs, and care coordinators
