Roles & Responsibilities

Provide compassionate bedside care to residents and patients on your unit
Administer medications and treatments safely and according to policy
Monitor and document patient status, notifying the RN/MD of changes
Collaborate with the interdisciplinary team to deliver coordinated care
Educate patients and families about care plans and self-care tasks
Maintain a safe, clean environment and adhere to infection control practices

Qualifications

Active LPN license in Pennsylvania required
Current CPR/BLS certification required
Practical Nursing program graduate with relevant clinical experience
Strong interpersonal skills and a team-oriented mindset
Experience in long-term care or med-surg settings is a plus

Education

Graduate of an accredited Practical Nursing (LPN/VN) program
High school diploma or GED

Knowledge & Skills

Patient assessment and basic clinical skills
Medication administration and documentation
Comfortable using electronic health records (EHR) for charting
Infection control practices and safety protocols
Clear communication with patients, families, and care teams
